Roles & Responsibilities
- An experienced professional who is insightful, resourceful, responsible and willing to mentor and supervise a team of social workers, social work associate/caseworker(s).
- Maintain a caseload as assigned by the Head of Home and provide individual casework, group-work intervention action plans including behavioural management of residents in care.
- Develop and facilitate group work for residents with the social team
- Monitor the residents’ progress in the Home and develop an Individualised Care Plan for each resident to work towards the eventual rehabilitation and reintegration into the community.
- Mobilise community resources and serve as a link between the resident and these resource systems as necessary.
- Coordinate social services through the case management approach by working with other professionals and non-professionals significantly involved in the care of the resident.
- Conduct home visit for a safety assessment of the home leave environment.
- Prepare and vet all internal and external reports (ICP, Review Committee, Discharge, etc) and attend relevant meetings
- Assist and oversee the development and the implementation of programmes/events relating to residents’ rehabilitation, wellbeing and volunteers’ management. Conduct evaluation of such programmes/ events and suggest areas of improvement, if necessary.
- Keep and maintain up-to-date records, reports and reviews of such programmes/events.
Requirements
- Must possess Degree in Social Work/Graduate Diploma in Social Work
- Must be willing to work with male destitute persons with Psychiatric conditions living in an institution
- Registered with SASW Accreditation Board is preferred with at least 5 years of experience
- Good report writing and interpersonal skills
- A good team player
- Demonstrate expertise in Casework, which involves end-to-end engagement with stakeholders, case assessment, goal-setting, the design, implementation and review of intervention plans, and documentation of case information. Play a contributory role in Group Work, Community Work and Programme Development in the organisation
