- Deliver accurate and evidence-based medication information and clinical guidance to healthcare professionals and patients. Support the development and administrative tasks related to Pharmacy and Therapeutics (P&T) Committee activities, such as:

- Composing comprehensive drug evaluation reports to support changes to the formulary
- Assisting in establishing systems for medication utilization
- Disseminating updates on formulary changes to clinical staff
- Monitoring and analyzing data on medication usage trends and adverse drug events
- Collaborate with internal and external departments to ensure the efficient and safe management of medication use throughout the hospital.
- Contribute to drug information and medication safety initiatives led by the Pharmacy department.
- Participate in the education and supervision of pharmacy students and pre-registration pharmacists within the drug information service.
Requirements:
- Registered under AHPC
- Min 3 year of relevant working experience
- Open to help in inpatient settings when needed
