Job Overview:
The Social Worker / Case Worker plays a vital role in facilitating positive change and improving the lives of individuals, families, and communities facing transitional challenges. This is achieved through a community-centric approach and the delivery of targeted interventions, including but not limited to social work services, home ownership support, livelihood development, and community mobilisation. The ultimate aim is to empower communities to break the cycle of poverty and achieve sustainable progress.
Working collaboratively with community members and ecosystem partners, the Social Worker / Case Worker supports socially disadvantaged, low-income families in realising their aspirations - particularly the goal of attaining and sustaining home ownership.
He/She will maintain a high standard of professionalism and be accountable to the client, the community, and the organisation, to ensure that intervention plans are developed and implemented with sound professional judgment, guided by best practices and ethical standards.
Responsibilities:
Oversee the Implementation of Code of Social Work Practice
- Oversee the implementation of BPSS, FAST in the context of Social Service Net (SSNet) for assigned Supervisees
- This includes delivering the necessary interventions and management related to the ComLink+ Progress Packages
- Maintain accurate and confidential family records, documenting progress, interventions and outcomes, on the required platforms such as (but not limited to) SSNet, CaseConnect, and Family Transformation Management System (FTMS)
Deliver Quality Community Centric Casework and Counselling Services to Clients
- Manage cases (i.e. from intake, assessment, documentation, referrals and closure)
- Attend networking and case management meetings with key stakeholders
- Conduct relevant assessments (FAST, BPSS, any other financial or risk assessments)
- Conduct home visits, Community Engagement and Networking sessions
- Provide case supervision, guidance and advice to support staff, volunteers and student interns
- Develop and review appropriate intervention strategies
- Develop, maintain and review the organisation’s casework and counselling system and SSNet
- Provision of social work administration supervision and support
- Initiate case conference with internal staff and external organisations
Deliver Specialist Interventions to Enable Families to Achieve Poverty Transformation
- Provide guidance specialist interventions to families, including conducting comprehensive assessments, developing action plans, setting goals, and making progress towards achieving home ownership and/or good livelihood and/or debt management and clearance
- Implementation of Home Ownership (KeyStart) and Good Livelihood Programmes, such as enrolling members into the respective programmes where relevant, and conducting the necessary activities e.g. Basic Habitation and processes e.g. seeking approval for Access Fund Grant, Basic Habitation Grant, Livelihood Grant, etc.
- Connect families with relevant resources, government assistance programs, and community support service, and work closely with partners, to address the needs of families to enable poverty transformation
Deliver Effective Programmes, including but not limited to Groupwork and Community Outreach
- Work closely with Senior Social Worker, Lead Social Worker and/or Executive Director to plan a holistic suite of programmes and services that are customised to the needs of the clients in the immediate area of influence/ community
- Identify assets, resources (manpower, budget, partners & etc) needed for the delivery of programmes
- Conduct community conversations, group work sessions and ensure smooth implementation of programmes
- Evaluate the effectiveness of the programmes
Requirements:
- Believe that people and communities have intrinsic strengths and resources that can be harnessed for the betterment of family and community life
- Knowledge and experience with social work interventions, social policies, and community engagement will be an added advantage
- Degree or Post Graduate Diploma in Social Work or related disciplines from recognised universities
- Strong analytical, writing, communication, and interpersonal skills Fresh graduates are welcome to apply
