The incumbent plays an important role in providing holistic care to our patients beyond rehabilitation in our centers.
Job Responsibilities
- Identify, promote and implement activities and wellness programs at the community dialysis center.
- Manage screenings, physical assessments, data compilation, analysis, and recommendations for follow-ups for dialysis patients.
- Conduct regular reviews to monitor patients' compliance and progression in the exercise program and ensure care plans are followed through.
- Maintain exercise and program equipment and ensures regular disinfection and servicing.
- Manage patients’ attendance to ensure accurate and timely documentation to maintain consistent participation.
- Collaborate with Clinical Services Team to address any barriers to participation and ensures patients receive the necessary support to continue their wellness journey.
- Assist in organizing volunteer events and outings to empower seniors and encourage engagement.
- Any other matters as required.
Job Requirements
- Minimum Diploma in Health Sciences (Health Management) or other relevant courses (e.g., sports sciences, therapy aide, community care associate).
- Relevant working experience in a healthcare setting is advantageous.
- Strong interpersonal skills and proficient in Microsoft Office.
- Positive and committed attitude with a strong desire to serve the community.
- Highly organized, detail-oriented, and multitasking ability.
