Job Description:
- Provide safe, personalized nursing care and maintain high documentation and infection control standards.
- Manage complex patient care using clinical skills and critical thinking.
- Continuously monitor and evaluate patient conditions, implement care plans, and support families.
- Collaborate with physicians and coordinate care, serving as a liaison between families and healthcare providers.
- Build rapport with patients and assess home needs for training and equipment.
- Ensure availability of necessary resources and follow up on patient care for continuity.
- Administer medications and perform wound dressings as needed.
- Maintain accurate patient records and compile data on referrals and attendances.
- Participate in Multidisciplinary Team meetings to ensure coordinated care.
- Implement safety measures and comply with standards for drug and supply management.
- Uphold patient confidentiality and manage equipment and supplies efficiently.
- Address public inquiries professionally.
- Follow organizational policies and Singapore Nursing Board standards, staying updated on industry guidelines.
- Maintain a clean and organized workplace.
Job Requirements:
- Diploma in Nursing from a recognized local Polytechnic, registered with the Singapore Nursing Board.
- Experienced in homecare nursing is advantageous.
- Oncology/ Cardiology/ Emergency/ Inpatient nursing experience is preferred.
- Proficient in IT and relevant systems.
- Passion for exceptional customer service to diverse stakeholders.
- Strong attention to detail and accuracy.
- Excellent interpersonal and communication skills.
- Effective time management.
- Team player with a collaborative attitude.
